The Neurax Worm is a type of disease that belongs to the group of Special Plagues, it possesses traits that can modify the internal chemistry of the human brain, allowing it to alter the behaviour of infected people. Among these alterations, the Neurax Worm can induce aggressive behaviour as well as hallucinations and psychosis, in addition, this pathogen can acquire lethal symptoms involving the generation of brain damage causing haemorrhages.
Another key factor is that Neurax Worm transmissions are different from their counterparts in the standard plagues, which can increase the rate of disease severity. In addition, the pathogen possesses traits that help increase its Locomotion which allows the worm to move from host to host more easily and subsequently unlock the Air and Water branch of transmissions.
Another unique aspect of this special plague is that it has two different endings, such as ‘Assuming Direct Control’ which involves the enslavement of all humanity by using Transcendence, which allows the worm to take total control of the minds of the infected. The other ending is called ‘Worm Food’ which involves the acquisition of lethal symptoms to extinguish the entire population of the world, similar to how the game is won using Standard Plagues.

Report published after research advances indicating to the public that Neurax Worm possesses abilities that can alter the behaviour of the infected population.
Neurax Worm has several transmissions responsible for the spread of Eggs, which may be involuntarily expelled by the initial host to reach new ones. Combined with the exclusive Water transmissions of this pathogen, these eggs can spread rapidly in unsanitised water, potentially dangerous in poor countries. On the other hand, the Air transmissions also have their variations for Neurax Worm, where the pathogen can intentionally dehydrate to become lighter and take advantage of air currents to spread to areas far away, additionally the latest stage of the airborne variant for Neurax Worm can help the plague survive inside air conditioning vents, which gives the disease a bit more of an advantage in spreading in generally rich areas with a hot climate.
Unlike other plagues, Neurax Worm has only one initial symptom which is Neural Breach, it allows the pathogen to infiltrate the individual's brain through the bloodstream, as this trait evolves, the affected population will develop eye dystonia. The next symptom is Cerebral Tendrils, which helps Neurax Worm to partially interfere with brain activity and unlock more symptom branches.
Among the possible branches of symptoms that Neurax Worm can develop are traits that can alter human perception of the pathogen including Adoration, Devotion and Transcendence, where the Worm emits different types of hormones that tend to produce a good feeling in the host's brain such as Dopamine and Serotonin which are closely related to changing the emotions of the patient making them consider reducing efforts to find a cure.
Regarding Neurax Worm's special abilities, this pathogen possesses the traits of Trojan Planes that allow it to control the people in charge of a flight, where they can be redirected to a specific country. The player can use this special ability to infect healthy countries that had previously shut down their transport system, in addition, the player can maximise the effects of these Trojan Planes in the target country by using Trojan Roamers to help infect more people in the territory or alternatively Trojan Destroyers to affect public order in the country.
Santa's Little Helper

In December 2014, Ndemic released Mutation 10 which officially includes Santa's Little Helper scenario,[4] where Neurax Worm serves a different function than the main game where the world is relatively much darker and gloomier where no one is allowed to have fun or even take international flights. The disease will have to alter the minds of the human population to convince them to opt for a better lifestyle.
Trivia
- The Neurax Worm is the only Special Plague that doesn't spawn a unique organisation dedicated to its destruction.
- The pathogen is partially based off of Toxoplasma gondii while the disease it causes is inspired by an episode from the "Futurama" TV series.[5]
- It lacks special genes and uses ones from the original game instead.
- The Neurax Worm appears to be an unofficial mascot of Plague Inc. On Ndemic's Twitter, many pictures related to the game have a plush Neurax Worm in them.
- There is also a special Neurax Worm that appears as an actual mascot for the game named "Neurie." It appears to be a Christmas tree ornament modelled after the icon for the Santa's Little Helper scenario.
- Due to these reasons adding the fact Neurax Worm's old look is cuter than the Evolved version, the worm became the most popular plague type in the game, going as far as representing the game itself along with the biohazard Logo.

Notes
In order to obtain the "Worm Food" achievement, the player needs to eradicate all of humanity with this plague without the use of Transcendence, and therefore it requires killing the infected population using symptoms such as Suicide, Despair and Aggression.
