Also, I think that they should have versions of (any) scenarios where the scenario is for a disease that (effectivly, anyway) doesn't exsist anymore, and do go back to when it DID exsist, including the Americas being missing for the black death, and some people (like those who DID survive the black death, and the gene that makes them imune to the black death also makes them imune to HIV) being "imune" to it, and you having to evolve to overcome that. Also, there is (du) no cure. For smallpox: it's time period, and you can even go to 1-3 or 4 decades before it was eridicated (the only disease TO have gotten eradicated in real life) and when it reaches about the same date, they'll release the vaccine, unless you've slowed them down enough. :)
