Patient Zero is the first patient that has been exposed to the player's plague type and is often mentioned in certain plague types/scenarios, such as Necroa Virus, Frozen Virus, Black Death, Smallpox, Nipah Virus, and Swine Flu. In the Necroa Virus, more specifically in Chernobyl and DarkWater origin stories, Patient Zero is mentioned, unlike most plague types where the patient is ignored instead.

Role

The main role of patient zero is to exponentially boost the cure/vaccine research progress for the disease when identified.

Main Game

Patient Zero is the first person infected by a plague, and once the plague gets out of control, the CDC may conduct a manhunt in order to find them and have them studied. The achievement Patient Who? involves preventing CDC to identify Patient Zero.

Cure Mode

In this mode, the player can find Patient Zero, if they direct the research team to the origin country. Eventually, the patient zero is found, this discovery will help accelerate vaccine research.

Trivia

  • In an E3 Trailer of Plague Inc., a doctor talks about Patient Zero, and that she had been infected with a strain of PAX-12 (the default name for all plague types excluding the Simian Flu's ALZ-113, Shadow Plague's Nox Eternis, and Cure Mode's PAX-19). However, Patient Zero has no noticeable gender in-game, nor has it been emphasized anywhere else.
